Medicine Name
Bifendate Pills (Lianben Shuangzhi Diwan)

Ingredients
Bifendate

Properties
It is a white crystalline powder, almost insoluble in water or ethanol, and easily soluble in chloroform. The medication is presented as sugar-coated drops.

Indications
It is clinically used for patients with chronic persistent hepatitis accompanied by elevated ALT levels. It can also be used for ALT elevations caused by chemical poisons and drugs.

Dosage and Administration
Oral: Drop pills, 5 pills each time, 3 times daily. If necessary, 6-10 pills each time, 3 times daily, for 3 months. After ALT normalizes, the dosage is reduced to 5 pills each time, 3 times daily, for another 3 months. For children: 0.5 mg/kg, 3 times daily, for 3-6 months.

Adverse Reactions
In individual cases, dry mouth, mild nausea, and occasional rash may occur. These symptoms usually disappear after adding anti-allergic drugs.

Contraindications
It is contraindicated for patients who are allergic to this product, patients with decompensated cirrhosis, and pregnant and lactating women.

Precautions

  1. In a few patients, ALT levels may rise during medication, but increasing the dosage can lower them. After discontinuation, ALT levels may rebound in some patients, but continuing medication remains effective.
  2. In individual patients, jaundice and worsening of the condition may occur during medication, and the drug should be discontinued.

Special Populations
Children: The dosage should be appropriately reduced.
Pregnant and Lactating Women: This product is contraindicated.
Elderly: Elderly patients should use this product with caution.

Drug Interactions
Concurrent use with inositol can reduce the rebound phenomenon of ALT reduction caused by this product.

Pharmacological Effects
This product is a domestically developed liver-protecting and enzyme-lowering drug for hepatitis, which is an intermediate in the synthesis of schizandrin C. In mice, oral administration of 150-200 mg/kg of this product can alleviate liver damage and ALT elevation caused by carbon tetrachloride. It inhibits microsomal lipid peroxidation and the metabolic conversion of carbon tetrachloride to carbon monoxide in the liver, and reduces the consumption of NADPH and oxygen during the metabolic process of carbon tetrachloride, thereby protecting the structure and function of hepatocyte biomembranes. This product can also reduce ALT elevation induced by prednisone and promote liver regeneration in mice with partial hepatectomy. The enzyme-lowering effect of this product is not by directly inhibiting the activity of serum and liver ALT or accelerating the inactivation of ALT in the blood, but may reflect reduced liver tissue damage. This product has a significant inductive effect on cytochrome P450 enzyme activity, thereby enhancing the detoxification ability for carbon tetrachloride and certain carcinogens. In some hepatitis patients, it can improve protein metabolism, increase albumin levels, and decrease globulin levels. However, it has no effect on the conversion of HbsAg and HbeAg to negative and cannot reduce enlarged liver and spleen.

Keep tightly closed and store in a dry place.
