Zuoquenuoyuntong Quecimi Pian
Ingredients
This product is a compound preparation, containing 6mg of Levonorgestrel and 3mg of Quinestrol per tablet.
Properties
This product is a film-coated tablet. After removing the coating, it appears white or almost white.
Indications
This product has an inhibitory effect on ovulation and is used as a long-acting oral contraceptive for women.
Dosage and Administration
- Take one tablet after lunch on the 5th day from the onset of menstruation, and take the second tablet 20 days later, or take one tablet on the 5th and 10th day of menstruation. Subsequently, take one tablet on the same day each month as the second dose, usually with withdrawal bleeding occurring 6 to 12 days after taking the tablet.
- When switching from a short-acting oral contraceptive to a long-acting contraceptive, take one long-acting contraceptive tablet the day after completing the 22 tablets of the short-acting contraceptive, and then continue to take one tablet on the same day each month thereafter.
Adverse Reactions
- In the first few cycles of taking the medication, some people may experience "early pregnancy-like" reactions such as dizziness, nausea, drowsiness, and vomiting, which occur mostly 8-12 hours after taking the medication. To mitigate and avoid these reactions, take the medication after lunch.
- Increased vaginal discharge may occur 3-6 weeks after starting the medication.
- A few individuals may experience excessive menstrual bleeding or amenorrhea.
- Other adverse reactions include stomach pain, edema, breast tenderness, and headaches.
Contraindications
Contraindicated in patients with uterine fibroids, breast lumps, impaired liver or kidney function, cardiovascular diseases, history of thrombosis, hypertension, diabetes, hyperthyroidism, mental illness or depression, and hyperlipidemia.
Precautions
- This product is suitable for couples living together long-term for contraception, with only one tablet needed per month, but it must be used under medical guidance as prescribed.
- Menstruation may occur 10 to 15 days after the initial dose, with the first two menstrual cycles being shortened, which is normal, and the third cycle returning to normal.
- During medication use, some individuals may experience vaginal bleeding due to insufficient estrogen levels. Additional Ethinyl Estradiol tablets can be taken, 1 tablet (0.005~0.01mg) each time, daily, or as directed by a physician.
- Regular physical examinations are recommended during medication use, and discontinue use immediately if abnormalities are found.
- If pregnancy is desired, discontinue use or adopt other contraceptive measures, and wait for six months before attempting to conceive.
- Contraindicated in patients with acute or chronic hepatitis, renal insufficiency, tumors, uterine fibroids, breast lumps, diabetes, thrombotic diseases, heart disease, lactating women, and severe hypertension.
Special Populations
Children: Not applicable.
Pregnancy and Lactation: Contraindicated in pregnant women. Lactating women may experience decreased milk production after taking this medication, so it should be started six months postpartum.
Elderly: Not specifically mentioned.
Drug Interactions
- Drugs affecting contraceptive efficacy: Antibiotics inhibit intestinal bacterial reproduction, reducing the decomposition of hormone conjugates and enterohepatic circulation; drugs inducing hepatic enzymes such as rifampin, hypnotics, and anticonvulsants, and antipyretics and analgesics; tricyclic antidepressants compete with this product for common metabolic enzymes in the liver.
- Drugs with efficacy affected when combined with contraceptives: Reduced effectiveness of antihypertensive drugs; reduced anticoagulant effect; decreased efficacy of antidiabetic drugs (such as insulin and oral hypoglycemic agents) in controlling diabetes; enhanced effect of tricyclic antidepressants.
Pharmacological Action
Quinestrol (Ethinyl Estradiol Cyclopentyl Ether) is a long-acting estrogen. After oral administration, it is absorbed through the gastrointestinal tract and stored in adipose tissue, slowly releasing Ethinyl Estradiol. By inhibiting the hypothalamic-pituitary-ovarian axis, it inhibits ovarian ovulation, achieving long-acting contraceptive efficacy. When combined with progesterone, it has a synergistic effect on inhibiting ovulation and can also cause endometrial transformation, exhibiting secretory changes, leading to withdrawal bleeding and forming cyclical changes. Taking one dose per month achieves a contraceptive rate of over 98%.
